I read this phrase, "sobrina carnal" in the context of royalty, but when I looked it up on google, I didn't see an explanation.

Wouldn't any cousin be a blood relative? Maybe unless there was a second marriage involved.

In this case, not cousin--it means niece. It means a niece of the first degree of relationship, much as 'primo carnal' means first cousin.

Brainlock on Cousin vs. niece, I know better.

Sobrina Carnal then would be a niece in the most direct way, the daughter of your blood sister or brother. You could have other nieces who would be the daughters of your spouse's siblings, I suppose, but then they would not be blood relatives, not a Sobrina Carnal to you.

[reply much as 'primo carnal' means first cousin.

Primo(a) hermano(a) is used far more commonly.

Greetings. I would like to point out a second layers of, ( Sobrinos). For example you have an, Uncle, he has a son, ( which is your cousin). Your cousin, ( sons, or daughters), are your second layer, ( Sobrinos). Gerry Zaragemca

Niece by blood relationship rather than "sobrina política" (niece by marriage).

¡Por fin! norteño has the exact correct answer. No complications, no necessity of tracking convoluted family trees! Thank you norteño!!!!

Then there are the degrees of removal: If your hermanos carnales have a children, those children are your sobrinos carnales. If those sobrinos carnales have children, those children are your sobrinos carnales - once removed. Etc., etc., etc. Sorry, but I don't know what the correct/legal term is in Spanish for degrees of removal.

Important NOTE: In México, your hermanos carnales are ALL the children of your mother, regardless of who the father is. Children of your father with some other mother are technically/¿legally? NOT your hermanos carnales. In the U.S. (English-speaking countries?) only the children of your mother AND your father are considered full siblings (hermanos carnales).
