Oaxaca City: Healthcare and Medical Insurance

I have a question for those of you who've retired in and around Oaxaca Ciudad.
What have you found to be the best arrangement for managing your healthcare as you grow older? Do you plan to use local medical facilities if a serious illness occurs or major medical procedure is needed? If so, do you carry IMSS insurance or have you opted for private insurance? Would you be ok going to Cruz Roja with a serious condition or is there another facility you prefer? What clinics, doctors have you had good or not so good experiance with?
How many of you would opt to return to your home country for major medical services and how many plan to use local services.
I'm really interested in hearing details about how you've arranged for your long term medical needs while living in Oaxaca, and also any stories or specific recommendations and advise you might have.
