";Health Certificate"; Required for FM3

To obtain an FM3, the Mexican Consulate in Dallas requires a "health certificate" from the family doctor. Has anyone had experience with this and if so, how did the health certificate read? Our doctor didn't have any idea, and it is pretty much impossible to get past the voice mail of the Mexican Consulate, and they don't seem to return calls when I do leave a message. <p>We were thinking the health certificate might be a letter stating we are in good health, have had our childhood immunizations, and have no communicable diseases. Does anyone know of anything else that might be included in this "certificate," what form it should take, and just how detailed it needs to be?<p>Thanks!

Thanks to you all for the info. We had the family doctor sign a letter stating that we are in good health, that we had no communicable diseases, and that we have had our childhood immunizations. Took the paperwork to Dallas this morning and got the FM-3 this afternoon. See you all in a few weeks!
