Entering the USA from Mexico or Canada....

When entering the USA,from Canada & Mexico,We have NEVER been asked if We have medical/health coverage.....Considering that the USA ,treats every entry as if applying to stay,it would seem pertinent to have coverage......or do they assume that no body would be so "Cavalier".
