Also, many medical services/honorarios are not subject to, or are exempt from, IVA. Physicians, I suppose should issue facturas in either case for other tax reporting purposes, such as ISR. Most insurance coverage or reimbursements will require facturas, but the problem here is the recipient of said services are the ones who pay the 16% IVA at the billing phase and in this case the clinic may want to add another 16% to an already high fee for the service, in order to receive a facture. If IVA is N/A, then I can't see why a facture couldn't be obtained, as I'm sure they have a pad of officially issued facturas by SAT, or as more commonly done y/o required now, sent electronically.
